Pennsylvania Advance Health Care Directive

This package contains a Pennsylvania Declaration Form. The Pennsylvania Declaration Form is a document which contains both a Living Will and a Power of Attorney for Health Care for use in Pennsylvania.

It enables a person (the “principal”) to name another individual as their agent (an “attorney in fact” or “health care agent”) to make health-care decisions for them if they become incapable of making their own decisions.
