Alabama Living Wills
The Alabama Advance Directive for Health Care includes a Power of Attorney for Health Care and a Living Will. This document allows you to make some decisions about future health care issues, by allowing you to select a health care agent, give health care instructions, or both. By using the Advance Directive, you can make decisions about life-sustaining procedures in the event of terminal condition, persistent vegetative state, or end-stage condition.
You can also use the advance directive to make any other health care decisions.This package contains:
(1) Information and Instruction for Alabama Advance Directive for Health Care (Power of Attorney for Health Care and Living Will);
(2) Alabama Advance Directive for Health Care (Power of Attorney for Health Care and Living Will) Form.This Alabama Advance Directive for Health Care includes a Power of Attorney for Health Care and a Living Will and is based on Alabama Statutes Section 22-8A-4. Excerpts from the Alabama Statutes relating to Advance Directive for Health Care are also included.
